Dear Health & Human Service Organization:
In order to be qualified as an Organization eligible to participate in the Charitable Contribution Preference
Program, organizations must:
(1) Be a 501 (c) (3) organization; AND,
(2) Directly provide health or human services to residents of Tallahassee/Leon County (1);
OR,
(3) Be either the United Way of the Big Bend or the United Partners for Human Services.
Here is an overview of the process required from an Organization in order to participate:
(1) Complete the Application and submit attachments.
(2) After you have been certified eligible and receive an ID and password for your account, please
keep your information updated.
(3) Reach agreement with a Vendor who would like to contribute to your organization.
(4) If donation is a product or service, receive 2 quotes, provided by the Vendor, to verify
the value of the donation.
(5) Complete the Verification Letter and submit it to UPHS at PO Box 10267, 32302 for certification.
The application process includes the following mandatory steps:

Attachments:
If you currently receive CHSP funding, please submit:
- Current CHSP Award Letter
If you do not currently receive CHSP funding, please submit:
- A copy of your most recent IRS form 990.
- A copy of your organizations most recent audit.
- A copy of your 501c3 designation letter.
- A Statement indicating the extent of your local direct service delivery.
(1) In order to be eligible, a significant portion of an organization's efforts and resources
must be utilized in providing services directly to the citizens of Tallahassee/Leon County.
Organizations whose primary mission is not to provide direct services will not be considered
eligible.
