THANK YOU for your interest in supporting organizations providing vitally important health and human services to the citizens of Tallahassee.

Here on the UPHS site, our job is to facilitate your efforts to identify an eligible organization.

You can identify eligible organizations four ways:

1. See a complete listing of ELIGIBLE ORGANIZATIONS.

2. Search for Organizations by NAME.

3. Search for Organizations by their SERVICE OR PRODUCT NEEDS.

4. Search for Organizations by KEYWORDS IN THEIR MISSION STATEMENT.

After you have identified the organization(s) that you are interested in supporting, it is up to you to contact that organization and talk with them about your interest in helping them.

Remember - all donations of products and/or services require two quotes to verify the value of the donation. It is the Vendor's responsibility to get the two quotes, not the Organization.

After the donation has been received by the Organization, along with the quotes (if applicable) they must complete a Contribution Verification Form, which states that they have received a donation and the value of that donation. The final step is for the Organization to forward the Contribution Verification Form to the United Partners for Human Services for final certification. UPHS will return the Contribution Verification Form to the Organization, where you can receive a copy of it for submission to the City.
